ASJJA Forms:

    1.  May be obtained from your Instructor/s;

     2. May be printed from this web site:

          A. Select the information and then select "print selection" from your printer options.

          B. Click print while at the desired form in the web page.  Please note that you will get at least 2 extra pages which includes the header and footer of the web page.

          C. Copy the form and paste it into your word processor (for example MS Word) by selecting what you want copied and then right mouse click to select copy.  Once in your word processor, select paste or again right mouse click and select paste.

All forms will need to be filled in neatly, and originals must be signed before giving them to your Instructor/s.

Classes Offered

Youth - $15 per month

Tuesdays & Thursdays: 6:00-7:00 p.m.
Saturdays: 9:00-10:00 a.m.

Adults: - $20 per month

Tuesdays & Thursdays: 6:00-8:00 p.m.
Saturdays: 9:00-11:00 a.m.

First month fees are free with USJA Membership. ($43.50) Family rates available.
